Care Connect is a major provider of community support services throughout Victoria, New South Wales and Queensland. Care Connect was established in 1994 as a not-for-profit organisation and is a registered charitable organisation. It is funded by both State and Commonwealth Governments through a variety of aged, disability and respite programs. Today, we support over 4,000 individuals in their decision to remain living independent in the community.
Care Connect's clients include:
- Frail aged individuals
- Adults with multiple and complex health issues which require input from a large number of services in a coordinated way
- Families with support needs due to caring for an individual with a disability or complex care needs
- Children with disabilities
Care Connect's client focused services include:
