- assessment of your support requirements
- preparation and management of support plans
- facilitation of community supports as well as personal and homecare services
- advocacy to promote your independence
- review of progresses to ensure alignment with outcomes
- service quality monitoring and modification to safeguard value and consistency.
Who is eligible for Care Connect programs?
Individuals accessing our services include:
- frail older adults with special/complex needs including dementia
- adults and children with an identified disability
- adults with multiple health concerns, including mental health issues
- families and carers who care for a child or adult with multiple and/or complex needs
- adults from culturally and linguistically diverse (CALD) backgrounds
- adults from the LGBTI community
- adults from Aboriginal and Torres Strait Islanders communities
- individuals transitioning from hospital to home
- veterans
