Community Aged Care Packages (VIC, NSW & QLD)

Community Aged Care Packages (CACP) are individually planned and coordinated packages of community aged care services designed to meet older peoples’ daily care needs in the community. CACPs are targeted at frail older people living in the community who require management of services due to their complex care needs.

Aboriginal Community Aged Care Package (ACACP)

The package offers a plan of support and care for frail Elders and those with age-related health conditions who have care needs and are living at home. All packages are individually planned and coordinated and help you to remain active in your own community.

Community Partnership Program (VIC)

The Community Partnership Program (CPP) provides support for older people from culturally and linguistically diverse (CALD) communities accessing information about aged care services. At Care Connect this is achieved by holding information sessions including seniors of the Macedonian and Turkish communities in the Western Metropolitan Region of Melbourne using interpreters and translated material.


Community Options – Culturally and Linguistically Diverse (CALD) Specific (NSW)

Community Options – CALD is a HACC-funded program providing case management and brokerage services to support people with complex needs (frail aged or disabled) to enhance their quality of life.

Community Options Program – Housing Support (NSW)

Community Options Program – Housing Support provides short to medium term flexible and co-ordinated case management and brokerage services for Health and Community Care (HACC) clients living in Sydney’s Inner West. The program uses comprehensive case management approaches to address complex needs and improve access to HACC service types.

ComPacks Program 

ComPacks is a six-week non-clinical care package comprising of case management and a combination of community services for people being discharged from a public hospital to safely return home.  Examples of support services that can be organised by the ComPacks Case Manager include assistance with personal care, domestic assistance, transport and social support. The ComPacks Program is also available at four sites in NSW as part of the early intervention hospital avoidance program Healthy at Home. The aim of this program is to provide community support and clinical care to avoid a hospital admission.

Extended Aged Care at Home (VIC, NSW & QLD)

The Extended Aged Care at Home (EACH) program was introduced as a pilot program in 1998 as a community alternative to high level residential care. It provides equivalent high level residential care to frail older people with complex care needs who wish to remain living in the community. The packages are flexible in content however the expectation is that a package would include qualified nursing input, particularly in the design and ongoing management of the package.

Extended Aged Care at Home Dementia (VIC, NSW & QLD)

The Extended Aged Care at Home Dementia (EACHD) program supports frail older people who have been assessed as experiencing behaviours of concern and psychological symptoms associated with dementia. These behaviours and symptoms significantly impact upon their ability to live independently in the community and also impact on their functional capacity to remain living at home.

Healthy at Home

The Healthy at Home program is a six-week preventative program designed for people aged over 65 years. It aims to keep people safe in their own homes by providing a combination of case management and community services that will help prevent hospitalisation.

Hospital Admissions Risk Program (VIC)

The Hospital Admissions Risk Program (HARP) was developed in the late 1990’s to address the increased demand on acute health care services. The HARP program identifies clients with chronic disease and at risk of, or already experiencing, frequent emergency presentations or hospital admission. The program provides individuals with appropriate alternative interventions.


Indigenous Social Support (NSW)

Indigenous Social Support is specifically funded for the Aboriginal and Torres Strait Islander Community and provides in-home and out-of-home support including social outings, shopping and friendly visiting.

Social Support Dementia Monitoring (NSW)

Social Support Dementia Monitoring provides in-home monitoring of people with dementia.

Dementia Options (NSW)

The Dementia Options Program supports people living with dementia by providing a comprehensive assessment of care needs. The assessment can be short term, long term or episodic and is individually tailored to meet your care requirements.
