Continence Program
Care Connect provides continence packages across the Eastern Metropolitan Region of Melbourne to assist children aged 5-15 years with disabilities who have bladder and/or bowel control issues. A comprehensive continence assessment is completed by a continence nurse inclusive of appropriate intervention. The provision for some continence aides is provided through the program as required.

Flexible Support Package and Linkages Package (VIC)
Flexible Support Package (FSP) and Linkages Package aim to assist children, individuals, families and carers access services and build informal social networks. The packages are funded by the Victorian Government and delivered by community organisations such as Care Connect in partnership with individuals, their families and carers. These packages complement existing services and assist the individual with a disability to remain living in the community with the aim of self managing their care.

Individual Support Packages (VIC)
Individual Support Packages (ISP) aim to assist individuals, families and carers to fund a range of disability supports to enable individuals to achieve their goals. The packages are funded by the Victorian Government and are delivered by community care organisations including Care Connect. This funding may complement existing informal arrangements provided by families and friends.

Independence at Home (NSW)
Independence at Home provides case management and brokerage service for adults with a disability who are living in the community and who are receiving support from their family. This program is suitable for people who need skills training in independent living inclusive of: shopping, cooking, budgeting and travel training. The program provides care planning, monitoring and coordination of services to enhance independence and living skills.

Disability Options (NSW)
The Disability Options Program supports people with a disability (and/or their carer) and helps them achieve greater independence and community engagement. The Disability Options Program provides flexible and tailored services delivered through the development of a coordinated plan, the provision of information, independent living training (shopping and grooming etc) and carer support.

My Future My Choice Program (VIC)
The My Future My Choice (MFMC) program is a state-funded program that aims to provide better living options for younger people in, or at risk of entry to, residential aged care facilities. Care Connect has been engaged to provide planning and facilitation support to people identified as eligible for this program. Care Connect has assisted many clients transition to more appropriate accommodation as well as set up supports and services for people to remain in residential aged care facilities. Although this is a time-limited program Care Connect has been engaged to continue to provide facilitation of My Future My Choice Individual Support Packages within the Southern Region.
Autism family support (VIC)
Initiated by a Knox City Council grant and developed by Care Connect, Spectrum Connections is a support group for families that have a child with Autism Spectrum Disorder.
Care Connect supports the group by providing:
- a place to meet and a safe environment in which people can share their experiences and their hopes
- facilitation of the group as required
- administrative support
- a range of information services including a Wiki page with a ‘What’s On’ section for the Outer East (Melbourne), an online library and a 24-hour chat line.
The success of this group lies in its ability to self-direct the information and services it requires. The outcome of this approach is a flexible and responsive program that meets the needs of its participants on an on-going basis. Care Connect plans to extend the support group across the Outer Eastern Metropolitan Region, starting with Croydon and Bayswater and extending into the Yarra Ranges in 2012.
Care Connect is currently supporting and funding a variety of activities and coordinates a website called ‘Spectrum Connections’ which provides a forum where families and carers can share knowledge and experience.
Spectrum Connections is funded by the Knox City Council.
