Case management is about responding to individual needs.
Our Case Managers are highly skilled health professionals and have significant experience in working with people as they move across the hospital, rehabilitation, residential and home environments.
We can offer a service whereby one worker is your key contact and can assist you with any of the following:
- Assessment of your service needs.
- Identifying goals and development of care plans.
- Provides information and assistance in identifying and accessing community supports.
- Advocating on your behalf.
- Arranging and coordinating services.
- Assisting with health promotion and education.
- Purchase of care services.
- After hours telephone support service.
Case management assistance can be for a short period, or longer term if required for people with complex, changing needs. In consultation with you and your family we can coordinate all your services and support your independence at home.
