Anna is a young woman who has an Acquired Brain Injury as a result of an accident when she was a teenager. While she is an active and engaging person she has an extremely poor short term memory which affects her ability to learn new things, problem solve, retain new information, and she gets easily confused and anxious when changes occur or unexpected situations arise.
Anna is studying at TAFE, participates in sporting and fitness activities, travels independently using public transport and likes to go out to the movies and dancing with friends. Anna moved out of home and now lives in house with two other young people with disabilities.
With the assistance of her Care Connect Case Manager the communication between Anna, her family and a large number of service providers is clear and constructive. The Case Manager coordinates and provides Anna with a monthly calendar which outlines her TAFE timetable, her personal appointments, her social activities and transport arrangements. It also shows the support workers that work with her on an almost daily basis. When this timetable is provided to Anna all details are recorded into her diary and a copy is also provided to family and service providers.
Anna also has an Issue Book which is designed to be used when Anna is feeling anxious or concerned; it is broken down into various topics and Anna records her thoughts and feelings. Anna then goes over any of her issues with her support workers and any suggested plan of action is recorded and results noted.
Training and information has been provided to family and service providers on the best ways to communicate and work with Anna taking into account her memory problems and levels of anxiety, and everyone is clear and consistent on directing Anna to use the strategies in place.
As part of Anna’s goals, the Case Manager has also assisted Anna in accessing social and recreation outings including regular social groups, an orienteering program, her local gym, dance parties and weekends away. Anna also accesses travel training as needed.
Care Connect Case Management service assists Anna to remain living in the community independently and also provides ongoing support and information to her family and services providers as issues and ongoing challenges arise.
