Case management is a collaborative process of assessment, planning, monitoring, facilitation and advocacy. Case management is designed for older individuals or adults and children with special needs.
The Care Connect Case Manager is the primary contact person for the client. They work with the client to identify their needs and coordinate the required services. On-going contact with the client ensures that suitable service is received.
Care Connect Case Managers work in accordance to the National Standards Practice for Case Management (Case Management Society Australia).

Confidentiality:
Your personal information will be treated confidentially, in accordance with the Commonwealth Privacy Act 1998 and the relevant State Privacy Acts, Principles and Guidelines.
Care Connect maintains strict confidentiality and respects religious and political beliefs and customs. Services can be engaged by clients, family members, guardians or funding providers and can be purchased for assessment only, short term or long term case management support.
Costs:
Fees will be charged according to your ability to pay. This will be discussed with you by your Case Manager. No one is ineligible for service due to inability to pay.
