The SAFTE Care program is for older people in the community who are at risk of hospitalisation. The program is currently available in the south-east Sydney areas of Kogarah, Rockdale, and St George, and offers the recipient a 6-week period of intensive support within the home in order to prevent hospitalisation. The SAFTE Care program is a joint initiative between Community Services (Care Connect) and Allied Health.
