Private Case Management

Private Case Management and Care Coordination provides individuals with a range of privately contracted user funded care services that are available within the community instead of waiting for a government funded package to become readily available. With the coordination of an experienced Care Connect Case Manager, the right type of assistance is chosen to help support you.

Private Case Management is available for any length of time, either short-term or long-term, depending on your individual needs. 

What can Private Case Management & Care Coordination offer?

  • Assistance to remain living at home with a level of support that you require
  • Advocacy and support on your behalf
  • The development of care plans that will identify and support your requirements
  • Assistance with health promotion and education
  • After-hours telephone support service
  • Referral to alternative support services
  • Residential Aged Care assistance (transition into or out of residential care, arrangements for shopping/banking/companionship services

More Information in identifying and accessing community supports such as:

  • High quality home or personal care
  • Allied health professional services e.g. physiotherapy and occupational therapy
  • Coordinating respite care, mental health and community services on your behalf
  • Accessing recreational, social,
  • Educational and employment services

Our Case Managers: 

We have highly skilled health professionals with significant experience in working with individuals as they move across the hospital, rehabilitation, residential, and home environments. Care Connect offers a Case Management service whereby one Case Manager is your key contact and support. Case Managers can assist you whilst you wait for a government funded care package and/or if you need a quick response to help you coordinate services in your home.

Your care services:

Care Connect in partnership with you, plans around your care needs and goals using a range of formal and informal services, so you get the best possible help.

Confidentiality:

Your personal information will be treated confidentially, in accordance with the Commonwealth Privacy Act 1998 and the relevant State Privacy Acts, Principles and Guidelines.

Care Connect maintains strict confidentiality and respects religious and political beliefs and customs. Services can be engaged by clients, family members, guardians or funding providers and can be purchased for assessment only, short term or long term case management support.

Costs:

Private Case Management is fully funded by you. Casemanagement is billed at an hourly rate. Other services you require can be purchased on your behalf and billed to you including an administration cost. A monthly invoice is supplied to you for all services provided. Services will only be purchased with your approval and consent.
