Care Connect is a not-for-profit organisation specialising in assessment, case management and brokerage services. With funding provided by both State and Commonwealth Governments, Care Connect operates in Victoria, New South Wales, Queensland and South Australia.

Formerly known as West Care Linkages, our organisation was established in Melbourne in October 1994, specifically to prevent the premature and inappropriate admission of frail aged and younger disabled persons to residential care (ie: nursing home and hostels).

Care Connect has grown from a staff of three people based in our Werribee office (Melbourne’s Western Region) to approximately 170 staff across the metropolitan region of Melbourne, in addition to Sydney, Ballina, Brisbane and Adelaide.  We pride ourselves on working collaboratively with service providers and community organisations in these regions, and believe that these partnerships have been fundamental to Care Connect’s growth and development.
