About this program:
A Community Aged Care Package (CACP) is a planned and coordinated package of community care services to assist a frail older person who needs management of services to remain living at home.
Who is eligible?
- A community aged care package is targeted at frail older people living in the community with:
- Complex care needs arising from interacting physical/medical, social and psychological needs.
- A need for skilled assessment and comprehensive management of service delivery.
- A need for services that are not provided/available from other community services.
- A preference to remain living at home with appropriate and reliable supports.
- A need for ongoing monitoring and review of changing care needs.
People who are eligible for the CACP program have been assessed by an Aged Care Assessment Team (ACAT) to be eligible for at least low level residential care, but wish to remain living in the community. The ACAT Assessment is free.
Which Services are provided?
Case Management: Within budget and practical constraints services that may be provided can include: showering assistance, home care, laundry, assistance with meals, in-home respite/social outings, and minor home maintenance. Services are jointly determined by the recipient or carer along with the Case Manager through a careful care planning process.
