Care Connect Dementia Options Program provides a case management and brokerage service for people with dementia who live alone. This includes those with long-standing disabilities who also have dementia. The program provides care planning, monitoring and coordination of brokered services to enhance independence and prevent premature admission to residential care.
People with a confirmed diagnosis of dementia who:
- Live alone.
- Live in Auburn, Holroyd and Parramatta LGAs.
- Have a need for regular monitoring and/or case management.
- Medication monitoring.
- Visits at mealtimes.
- Small group outings.
- Information and practical support.
- Carer support
