Care Connect’s Dementia Respite Program provides a flexible respite service for carers of people with dementia. The program provides care planning, monitoring and coordination of brokered services to support carers and maintain caring relationships for people with dementia. Carers of people with challenging behaviours will be given priority.
Carers of people with dementia who:
- Require respite.
- Live in Auburn, Holroyd, Parramatta, Blacktown and Baulkham Hills LGAs.
- Home based respite.
- Community-based respite.
- Information and practical help.
- Support for carers and families.
- Assistance to attend centre-based respite.
- Planned and flexible respite.
- Social outings.
