Care Connect DisAbility Options provides a short to medium term case management and brokerage service for people with a disability, to maximise their independence and community access. This program provides support to clients and their carers, especially ageing carers, through innovative and flexible care planning, and service provision.
Adults with a disability who:-
- Have complex needs
- Require short to medium term case management
- Live in Randwick, Waverly, Woollahra, Sydney, South Sydney and Botany LGAs
- Case management
- Information
- Flexible and individually tailored services
- Independent living training
- Carer support
