This HACC Funded Respite Program offers coordination of Respite Services which will be provided on a one to one individual basis.
Respite refers to services which strengthen and maintain the primary care relationships between a frail older person or a younger person with a disability and their carers through the provision of flexible and responsive alternatives to the usual care arrangements. The Program is preventative in focus, time limited, and generally provided on a planned basis.
Whilst providing a respite effect for carers which is appropriate to their needs and circumstances, services coordinated through this Program will endeavour to ensure that respite options have positive benefits and outcomes for the frail older person, or person with a disability.
Overnight respite cannot be provided in a centre-based or residential care setting.
This program offers co-ordination of respite support and is not a case management program. A fee for service applies.
Who is eligible:
Carers who:-
- Care for a person who is eligible for assistance under the HACC program.
- Require some planned respite.
- Live in Auburn, Holroyd, Parramatta, Blacktown and Baulkham Hills LGA’s.
What services can be provided:
- Home based respite.
- Support for carers and families.
- Planned respite options.
