Care Connect Independence at Home Program provides a case management and brokerage service for adults with a disability who are living in the community and are receiving support from their family. This program is suitable for people who are interested in independent living training within the home. This can include shopping, cooking, budgeting, time management and travel training. The program provides care planning, monitoring and coordination of brokered services to enhance independence and living skills.
- Adults with a disability who:
- Live in the community and receive support from (or live with) their family/carer.
- Would benefit from independent living training.
- Require case management support to enhance this independence.
- Do not require 24 hour supervision.
- Have low to medium support needs.
- Live in Randwick, Waverly, Woollahra, Sydney, South Sydney and Botany LGAs.
