Flexible Support Packages integrate key aspects of the Early Choices, Family Choice, Continuity of Care and the Making a Difference Programs to provide Case Management and /or discretionary funding for children and adults with a disability, their families and carers. All programs are under the auspice of Disability Services within the Department of Human Services.
Eligibility is according to program guidelines from the Department of Human Services.
