Cardinia Dementia Connections – Nobody can do it alone

To coincide with Dementia Action week, Care Connect care finder program is hosting an event in conjunction with local community organisations, to raise awareness of dementia.

At this event you will learn about services and supports available to people living with dementia and their carers, in Cardinia.

Where and when:

  • Tuesday 16th September
  • 10am – 12pm (doors open 9:30am)
  • Outlook Community Centre
  • 24 Toomuc Valley Rd Pakenham VIC 3810

This is a FREE community event, bookings are essential. Please call 1800 940 633 (free call) to reserve your place. A light morning tea will be provided following the information session.

Care Connect care finder program

The care finder program is funded by the Australian Government to help vulnerable older people navigate the aged care system and find support services to improve their quality of life. Care Connect care finders meet with you in person and can help with at any stage of your aged care journey; registering with My Aged Care, assessment support, accessing services for the first time and changing or finding new services or supports.

Cardinia Shire Council offer a range of services, events and activities to help people age well – to maintain physical and mental health, feel safe and secure, be involved in the community and stay connected with others.

Come and see the team for a copy of the latest Ageing Well Newsletter. The newsletter is a great source of information for our older residents. Published three time a year, it features stories about local residents, opportunities to participate in community and health and wellbeing articles.

Australian Multicultural Community Services

Learn about Australian Multicultural Community Services programs available including their Multicultural Dementia Care and Support Centres.  Their Multicultural Dementia Care & support Centres make a positive impact for the caregiver and the individual.

Services Australia

You, your family or a representative may be able to speak to one of our Aged Care Specialist Officers (ACSOs). This is a free service available to help you with your aged care matters. You can meet with an ACSO in-person in some of our service centres or through video chat from anywhere in Australia. They can help you by:

  • providing in-depth information on the different types of aged care services
  • checking if you’re eligible for government-funded services and making a referral for an aged care assessment
  • providing financial information about aged care services
  • helping you appoint a representative for My Aged Care
  • connecting you to local support services

Alfred Health | Carer Gateway 

    

Offer information, support and resources for carers and young carers (25 years and under) who provide ongoing unpaid personal care or support. Alfred Health provide Carer Gateway (Commonwealth funded) and Support for Carers (State funded) services.

This café style program brings together people living with dementia and their loved ones The café is a nurturing space that offers enrichment, support and understanding, where you can share experiences with others living with the challenges of dementia. Come along to enjoy activities, entertainment and refreshments.
