You may have heard "AT-HM" mentioned by your Care Manager or GP and wondered what it means - the Assistive Technology and Home Modifications Scheme.

The Assistive Technology and Home Modifications scheme helps older people get the support they need to stay safe and comfortable at home. This can include assistive technology and home modifications, like rails or equipment to make daily tasks easier. What is the Assistive Technology and Home Modifications Scheme (AT-HM)?

The AT-HM Scheme is part of the Support at Home program. It provides separate funding for equipment and home changes that help you live safely and independently. This funding sits outside your regular care budget, which means it does not reduce the services or hours you already receive.

Depending on your needs, the scheme can support:

  • equipment to help with daily tasks
  • changes to your home to improve safety and access

The goal is simple, to help you stay living comfortably in your own home for as long as possible.

What can the Assistive Technology and Home Modifications Scheme help with?

There are two main types of support available:

To access this support, you need to:

    • be part of the Support at Home program
    • have an aged care assessment (ACAT

Your eligibility is based on your individual needs, so not everyone will receive the same level of support. If you are not yet set up with My Aged Care or have not had an assessment, we can explain the steps and help you get started.

How much funding is available?

The amount of funding depends on the type of equipment or home changes you need and how complex they are. Funding is grouped into different levels, based on cost and complexity.

 

Tier 1 Low cost – up to $500/year Covers simple, ready-to-use items that don’t need a professional assessment. This might include things like non-slip mats, a basic grab rail or a walking stick.
Tier 2 Medium cost – up to $2,000/year Covers items or small modifications that need advice from a health professional, such as an occupational therapist. Examples include a shower chair, raised toilet seat or minor changes like widening a doorway.
Tier 3 High cost – up to $15,000 per year Covers more complex or higher-cost items and changes to the home. These require a formal assessment and prescription. Examples include stair lifts, platform lifts or larger bathroom updates.
