You may have heard "AT-HM" mentioned by your Care Manager or GP and wondered what it means - the Assistive Technology and Home Modifications Scheme.
The Assistive Technology and Home Modifications scheme helps older people get the support they need to stay safe and comfortable at home. This can include assistive technology and home modifications, like rails or equipment to make daily tasks easier. Care Connect can guide you through the process and help you access the support that’s right for you.
What is the Assistive Technology and Home Modifications Scheme (AT-HM)?
The AT-HM Scheme is part of the Support at Home program. It provides separate funding for equipment and home changes that help you live safely and independently. This funding sits outside your regular care budget, which means it does not reduce the services or hours you already receive.
Depending on your needs, the scheme can support:
- equipment to help with daily tasks
- changes to your home to improve safety and access
The goal is simple, to help you stay living comfortably in your own home for as long as possible.
