What is the End-of-Life Pathway?
The End-of-Life Pathway is part of the Support at Home program. It provides urgent funding to people who have 3 months or less to live and who want to remain at home. It is provided alongside the care you already receive from your doctor, hospital or palliative care team. This pathway is designed to keep you comfortable, supported and well cared for at home during this time.
