When you or someone close to you is approaching the end of life, there can be many decisions and arrangements to consider.

What is the End-of-Life Pathway?

The End-of-Life Pathway is part of the Support at Home program. It provides urgent funding to people who have 3 months or less to live and who want to remain at home. It is provided alongside the care you already receive from your doctor, hospital or palliative care team. This pathway is designed to keep you comfortable, supported and well cared for at home during this time.

Who is the End-of-Life Pathway for?

The End-of-Life Pathway may be right for you or a loved one if:

- you are aged 65 or over, or 50 or over for Aboriginal and Torres Strait Islander people
- a doctor or nurse practitioner has confirmed a limited life expectancy
- you wish to remain living at home
- you meet the aged care assessment requirements.

You do not need to already be receiving Support at Home services. The process is fast-tracked so support can begin as soon as possible.

What services are available through the End-of-life Pathway?

At this stage, care is focused on comfort, dignity and helping both you and your support people (family, carers) feel peaceful. Care Connect can help arrange the support you need at home, including:

Nursing care

and clinical support

Allied health services

physiotherapy, occupational therapy and nutrition support

Personal care

help with showering, dressing, grooming

Domestic assistance

cleaning, meal preparation, laundry

Transport to medical appointments

Social support and companionship

Respite and carer support

How much funding is available under the End-of-Life Pathway?

The End-of-Life Pathway provides up to $25,000 of funding over 12 weeks to pay for your in-home care.

Clinical services, like nursing and allied health, are fully funded by the government, however everyday living and independence services may involve a contribution payment. These contributions will depend on your income and assets.

We will always explain any costs clearly before services begin.

How do I access the End-of-Life Pathway?

Speak with your doctor or nurse practitioner

Ask them about the End-of-Life Pathway under Support at Home. They will need to complete a form confirming your diagnosis and expected life expectancy.
Your doctor provides the completed form to an aged care assessor

If you are not already registered with My Aged Care, Care Connect can help you with this step.
Receive Approval

Once approved, Care Connect will work with you to arrange services as quickly as possible.

If you are already receiving care, your existing services will continue. The End-of-Life Pathway provides additional support on top of what you already have.
