What is a Home Care Package?
A Home Care Package is a government subsidy provided to help you live at home as long as you want. The funds are provided to an approved home care provider, who coordinates a package of home and health care and case management services to meet your needs.
Home Care Packages are provided under a consumer directed care model, which means that the choice of how to spend your package funds is in your hands. You and your approved provider become partners in developing and managing the care you need.
At Care Connect we are experts in home care services, with more than 25 years’ experience helping people live at home by accessing the funding and services they need. We can help you get the most out of your Home Care Package, because we offer independent advice and guidance.

- Home Care Level 2 – a package to support people with low level care needs.
- Home Care Level 3 – a package to support people with intermediate care needs.
- Home Care Level 4 – a package to support people with high care needs.
