Often the best place to recover from a procedure or treatment in hospital is in the comfort of your own home, and transitional care is designed to allow you to do this. Transitional care is a coordinated package of services that includes nursing support, therapy, personal care and domestic assistance, and is focused on helping you recover and regain independence after a hospital stay.

With our large network of trusted providers we can connect you with the help you need. We are committed to making your recuperation after hospital the best possible experience.

ComPacks – NSW public hospital patients only

Funded by the NSW State Government, ComPacks is designed to support your safe return home from hospital so you can regain your independence and prevent readmission. The program involves putting community services in place for you for up to six weeks, including referrals to mainstream services. Your ComPacks program will start once you’re discharged from a participating New South Wales public hospital.

After returning home from hospital I received your ComPacks Program. The girls were so wonderful, they really understood my needs. After the program I was walking and much stronger.  I will always be grateful for what the girls did for me.
