Care Connect has more than 20 years’ experience supporting people with disability to live independently at home and stay connected to their community. Our purpose is to work alongside people, families and carers so they have the information, choice, control and services they need to live safely, with dignity, and to participate in community life.

Services that support inclusion and participation
We provide tailored disability support services for people of different ages, abilities and life stages, including children, young people, adults, older adults and carers. This includes helping people understand the disability support system, access the services they need, remain living in their own home, and participate in their community.
Responsive, person-centred support
We work with you to put a plan in place that reflects your rights, choices, goals and changing needs. Our role is to act promptly, provide clear information, and connect you with practical supports so you can continue doing the things that are important to you.

As an independent not-for-profit provider, Care Connect works with a network of quality, police-checked and accredited providers to connect people with reliable supports that meet their needs and represent value for money.

We help people understand the NDIS, their options and the supports available to them, so they can make informed decisions about services that enable independence, participation and inclusion.
