As a trusted not-for-profit provider, we’re committed to making quality nursing care accessible and dependable. Under the Support at Home program, approved nursing services are fully funded, so there is no co-contribution required.

What is in-home nursing care?

In-home nursing care is clinical care provided by nurses in your own home, instead of in a hospital or clinic. If you are recovering from surgery, living with a chronic condition, or need regular health checks and monitoring. Home nursing and care supports your health while helping you stay comfortable in familiar surroundings. If you are rebuilding strength and confidence after illness, injury or a hospital stay, our Restorative Care pathway may also be a helpful short-term support option.

Our in-home nursing care services

Care Connect offers a complete range of home nursing and care services, delivered by registered nurses rather than support workers. We can support you at home with:

Support to manage medicines safely at home, including medication reviews, blister pack support and administration by a registered nurse where clinically required.

Our nurses can complete clinical health assessments in your home, including baseline observations, health and wellbeing monitoring. Nurses work closely with your Care Partner, GP or specialist. This is a proactive way to maintain your health.

Funding your in-home nursing care

There are a number of ways to access at-home nursing services, and many are government-funded. Under Support at Home, nursing is classified as Clinical Care which means all costs come out of your approved Support at Home budget. This means you won’t need to make a personal financial contribution.  The Commonwealth Home Support Program (CHSP)and Out of Hospital Care Program suits lower-level needs. You can also choose to pay privately if you would like to start services sooner or prefer not to wait for an assessment.
