From our Referrals team to our Care Management team, Care Connect is committed to providing trusted advice and choice of services to help you stay living at home.

We are focused on taking the stress out of home care planning, and putting the control back in your hands. Your Care Connect team will give you trusted independent advice, services that are right for you to enable you to live in your home and community.

Referral Team

Our Referrals Team takes the time to listen and understand your individual circumstance. We work with you to arrange an application for government funding if you don’t have a Home Care Package. We connect you to a Care Manager if you have a Home Care Package.

Care Managers

Our Care Managers visit you in your home to understand your individual needs and help tailor your care plan, establish a budget and organise the delivery of services. They also provide support and guidance to keep you living in your home for as long as possible.

Support Coordination Team

The team set up and schedule your personalised set of services from providers and support you in an ongoing capacity to reschedule and/or change services with providers on your behalf.
