1. The challenge:

Recently, Darryl experienced deterioration in his health which led to a hospital admission and surgery on his liver. His treatment was successful, but his recovery is complex, requiring drainage tubes and some reduced mobility.

2. Solution without a Care Manager:

As Darryl was not able to seek the necessary additional care from their daughter to remain living independently at home, it is likely that Darryl may have had to explore options outside of independent living at least on a temporary basis.

3. The Care Manager difference:

Melissa was able to answer many questions for Darryl and simplify the next stage of his aged care journey. Darryl felt more energised and hopeful with Melissa’s personalised expertise.

Melissa understood Darryl’s practical challenges, between his reduced mobility and the recovery challenges that required additional support.

Melissa coordinated services that included assistance with the regular grocery shop and transport to medical appointments enabling him to feel safe, secure and more at ease as he continued to live at home.

Weekly cleaning services were scheduled that supported him to live in a clean, safe home whilst getting some much-needed rest and supporting his independence.
